Student Survey Procedures
When is the survey administered?
Please take roll and if any students are absent, provide a second survey opportunity in an attempt to have 100% of enrolled students surveyed. Surveys must be completed by June 30 of each year.
Who completes the survey?
Provide one survey to every enrolled student to complete (preferably in a quiet, whole-group setting). Ask students to answer the questions independently, allow adequate time to complete, and collect the surveys.
Teacher Instructions
Prior to handing out the survey, please mark the box above identifying the after-school program for which you are administering the survey, the location of the program, and the class/activity.
Project Director/Data Specialist Instructions
Once the student survey has been completed, please use the Student Survey Aggregate Template to consolidate all the information into a single document. Submit the completed aggregate file no later than June 30th each year.
