Emergency Readiness Plan

Every 21CCLC center must document and maintain a site-specific Emergency Readiness Plan, which must be submitted to CEB 21CCLC at least 30 days prior to program start and must be available to all staff upon request. The plan must specify the emergency procedures applicable to the out of school time program. The plan must also include:

  1. Schedule for training out of school time staff on policies and procedures
  2. Schedule for emergency drill practice during out of school time
  3. Contact information of key staff on campus during the out of school time program
  4. Site map containing an evacuation plan for the out of school time program location

Note: A copy of the school's HIDOE required Emergency Preparedness Plan may be used. The plan must include emergency procedures in regard to students and staff participating in out of school time programming on campus.

Site Specific First Aid / CPR / AED Certification

Programs must ensure that at least one staff member with current certifications in First Aid, Cardiopulmonary Resuscitation (CPR with a face-to-face component), and Automated External Defibrillator (AED) is present on site at all times at each program site. Certification evidence must be uploaded for each site and will be reviewed as part of CEB 21CCLC monitoring. The CEB 21CCLC Team will verify all submissions. The document must contain the following to verify staff members:

  • The first & last name of the certified Staff members
  • Certificate Number or QR Code per staff member
  • The name of the organization issuing the certifications
  • The site name associated with the certified member
  • Expiration date is optional. However, the same form is a great way to keep track of certified staff

Note: Subgrantees may compile and submit a single document for multiple sites. The subgrantees may create & submit a table, chart, etc. that includes the following:

Snack/Meal Evidence

All 21CCLC programs must offer nutritious snacks/meals that meet the requirements of the
USDA guidelines for afterschool snacks and summer meal supplements, as well as the Hawaii
Nutrition guidelines.
At a minimum, programs must provide the following supplemental snacks/meals:

  • Afterschool: daily nutritious snack; daily dinner if program extends 5 hours or more
  • Before school: daily nutritious breakfast
  • Morning programs during non-school days: 1 meal and 1 snack
  • Afternoon programs during non-school days: 1 meal and 1 snack
  • Day-long program or any program lasting 6 hours or more during non-school days:
    2 meals

While these are minimum requirements, snacks/meals cannot be purchased with 21CCLC grant funds and must come from other resources.
Subgrantees are encouraged to utilize partner donations or participate in the USDA Food and Nutrition Service programs that provide meals and snacks to students and families outside of the school day. Students or their families cannot be charged for any costs associated with supplemental snacks/meals directly (e.g., fees) or indirectly (e.g., requests for donations). (Requiring parents to provide snacks/meals as a condition of student participation is not
acceptable.) For more information on meals and snacks and how to acquire funding:

Note: The following are some acceptable forms of snack | meal evidence:

  • A digital copy of the school's "Signed & Approved" Afterschool Snack Program Application
  • Award letters that indicate private funding for specific sites

School Calendar

  • The school calendar will be used as a reference point for data collection, site visit scheduling, and other program related tasks. To ensure data accuracy & to avoid scheduling conflicts, upload the participating school's calendar.
  • Note: HIDOE School Year is not acceptable due to the varying waiver days & other non-student days that each school formulates within it's own particular school year calendar.
