HIDOE schools interested in participating in the United States Department of Agriculture (USDA) Afterschool Snack Program (ASP) for school year 2027-2028 shall complete and submit the following form with original signatures to the School Food Services Branch (SFSB) for approval:
• Application for the ASP; and
• Schools with multiple programs must submit a separate application for each program requesting snacks.
Application Deadline: Please complete the attached application and email it to sfsb@k12.hi.us by close of business on TBD.
