Site Specific First Aid / CPR / AED Certification
Programs must ensure that at least one staff member with current certifications in First Aid, Cardiopulmonary Resuscitation (CPR with a face-to-face component), and Automated External Defibrillator (AED) is present on site at all times at each program site. Certification evidence must be uploaded for each site and will be reviewed as part of CEB 21CCLC monitoring. The CEB 21CCLC Team will verify all submissions. The document must contain the following to verify staff members:
- The first & last name of the certified Staff members
- Certificate Number or QR Code per staff member
- The name of the organization issuing the certifications
- The site name associated with the certified members
- Expiration date is optional. However, the same form is a great way to keep track of certified staff
Note: Subgrantees may compile and submit a single document for multiple sites. The subgrantees may create & submit a table, chart, etc. that includes the following:
